There is diffuse swelling of the hepatocytes, proliferation of Kupffer cells lining the sinusoids and infiltration of the portal areas by lymphocytes and monocytes.
The liver parenchympa may have a diffusely decreased echogenicity with accentuated brightness of the portal triads, periportal cuffing .
Hepatomegaly and thickening (edematous) of the gallbladder wall are associated findings.

CHRONIC HEPATITIS
In most cases the liver appears normal.
In chronic hepatitis are also sonographically normal.
When Chronic hepatitis and cirrhosis develops, sonography may demonstrate a coarsened echotexture and other morphologic changes of cirrhosis.
HOW TO DEFINE HEPATITIS
In acute hepatitis :
